Petition for
Exception into the Teacher Education Program
(PDF) - Any
student wishing to appeal a program admission decision, an internship
admission decision, or a certification recommendation decision must
make such an appeal in accordance with the policies and procedures established
by the Teacher Education Committee (TEC).

One-Time Waiver Form (PDF) - This
form is for allowing a student into a class before their official acceptance
into the College of Education. This form can be used one time.
Undergraduate Course Substitution
Form
(PDF) - This form is
for substituting one class for another to fill a required prerequisite for
graduation.
Approval to Transfer
Credit Form/Transient Study Credit (PDF)-
This form serves as written approval from the student's advisor and
dean's designee prior to registration for courses taken at another
institution. See Undergraduate Catalog for additional
information.
Change of Major Form
(PDF)-
This process begins in your college advising office.
For students with majors in the College of Education, please submit
the completed form to Student Academic Services (144 Withers).
